How to Order Your Phoenix Bear

Donate $15 to be placed on the waitlist for a bear & help cover shipping fees (ships within the USA only at this time)
After submission of form is complete, you will receive a confirmation email with your Phoenix Bears order number (please keep this for future reference)
IF you choose an Express Pass option, you will need to promptly purchase your base bear from our “Choose Your Base Bear” Amazon WishList and have it sent directly to us. Be sure to include your Phoenix Bears order number. You will not be placed on the “Express Pass” until your base bear is received.

Custom weighted Phoenix Bears are free to bereaved parents. There is only a required $15 donation that helps cover shipping fees. Every Phoenix Bear ordered comes with a blanket attached to it’s hand and a digital copy of your baby’s personalized Certificate (certificate will include a picture of your Phoenix Bear on it). Other options are angel wings and personalized embroidery. You can opt out of any of these personalized options that you do not desire to have included.

Your base bear will be chosen for you according to the weight of your baby and the available donated bears.
If you want to choose your own base bear, you will choose & purchase your bear through our Amazon Wishlist after completing the form & receiving your Phoenix Bear order number. If you choose to do this, the cost of the base bear will be your responsibility and you will not be reimbursed for this expense. Choosing your own Base Bear will put you on the “Express Pass” which will reduce your time on our waitlist.

IMPORTANT: Please note: We only ship within the USA at this time. For multiple losses, please fill out one form for each baby if you want a bear for each baby. If you’d prefer one angel bear holding multiple 2.5″ baby bears, please fill out one form and put that information in the additional information section.
Phoenix Bears can be weighted from 0.7 oz all the way up to 12 lbs. There is also an option for unweighted bears (just put 0oz in the weight option). If your baby was heavier than 12 lbs and up to 20 lbs, CLICK HERE to view your options to order a Bigger Bear.

Only the bereaved parent can order a Phoenix Bear. This is to make sure nobody receives a duplicate bear. *
